Individual Dental Insurance

It is very difficult to find a good Individual Dental Insurance program. Traditionally dental insurance has only been only available in the employer group format. Until now!

Executive Benefit Plans partnered with our sister company, Universal Health Systems, Inc., @ DentalQuotes.com to offer an individual dental product online, in all 50 states. A slightly different individual dental product is also available in certain jurisdictions.

Our plans allow you to choose any dentist for your convenience. As an added enhancement to our dental plans you also have the option to save more money by using our Dental Network.

The Silver Plan – A [50% - 50% - 50% plan] the Silver Plan is a great plan for individuals and families - offering a full compliment of benefits for all covered dental procedures. A deductible is not required for any Type I - Preventive Diagnostic Services,* which are reimbursed at 50%, but a $50 contract year deductible is required for Type II – Basic Restorative Services,* which are reimbursed at 50%, or Type III - Major Services** which are also reimbursed at 50%. Please note this plan provides 50% reimbursement for all covered dental procedures including Type IV - Orthodontia Benefits,*** to the Reasonable and Customary limits of the area where you receive service.

The Silver Plan offers each insured person 50% benefits for periodic exams, cleanings, x-rays, and fluoride treatments, and basic restorative benefits for fillings, extractions and oral surgery after an initial 30-day waiting period from the plan effective date. The plan offers 50% reimbursement for major restorative benefits including Root Canals, Crowns, Bridges, Dentures, Partials, and Periodontal Surgery after an initial 12-month waiting period. The Orthodontia benefit is available for children between the ages of 6 and 18 after the 2nd year of the policy. The benefit is 50% to a separate maximum of $1,000. Example: if the Ortho cost is $2,200 the policy will pay $1,000 (50% to $1,000). Click Here For Rates

The Bronze Plan – An [80% - 60% - 0% plan] the Bronze Plan is a great plan for families with small children - offering an affordable alternative to the Silver Plan. After a $35 contract year deductible is satisfied this plan provides benefits at 80% for the important Type I - Preventive Diagnostic Services,* and at 60% for the Type II – Basic Restorative Services,** to the Reasonable and Customary limits of the area where you receive service.

The Bronze plan offers each insured person benefits reimbursement at 80% for periodic exams, cleanings, x-rays, and fluoride treatments*, and at 60% for basic restorative benefits like fillings, extractions and oral surgery**. While this plan will not provide any benefits for Type III - Major Services, or Orthodontia Benefits, it offers a dental plan at a lower cost than the Silver Plan.
